| Local chapters of the USAWOA award the JROTC Medal for Distinguished Service to the outstanding Cadets participating in the JROTC program. Along with the medal, a Citation for Distinguished Service can be printed for each recipient after your order is confirmed. The citation is to accompany the JROTC Medal for Distinguished Service. |

To Fill-in and Print the completed form... |

- Click on the form shown to the right. |

- When the form opens, press the TAB key to place the cursor below the word "TO" |
|
- Enter the awardee's name, up to 40 characters. Press the TAB key |
|
- Eenter the School year in the form of YYYY. Press the TAB key |
|
- Enter the date in the form of Day Month Year. Press ENTER |
|
- Right click and Select PRINT |
|
- Select your printer. Make any printer setting changes. Click OK to print the document |

- Click the Close Box (X) to close the form and return to the instructions page |
